Release Agreement
I acknowledge that soccer is an inherently dangerous sport in which I participate at my own risk. In consideration of accepting my application, I, intending to be legally bound, do hereby, for myself, my heirs, representatives, executors, and administrators, waive, release and forever discharge the San Luis Obispo High School, the City of San Luis Obispo, and their agents, officers, directors, contractors and employees from all liability, claim, loss, cost or expense, whether or not attributable to the actions or omissions of any such person, arising out of accidents, illnesses, injuries, and other consequences suffered by me while participating in SLO Tigers 7 v 7 tournament or resulting directly or indirectly from said activities. I affirm that I am in good physical condition. I understand that San Luis Obispo High School does not carry medical insurance and that I am responsible for my own insurance coverage. This above verified by my signature below. THIS IS A RELEASE OF YOUR RIGHTS. READ CAREFULLY BEFORE SIGNING.
