1. Registration Paperwork – We need:

2 copies of the US Club registration paperwork. You can download a copy of the registration paperwork here http://www.slosoccer.com/SLOSC/documents/PLAYER-REGIS_MED%20AUTH%20FORM-1.doc <http://www.usclubsoccer.org/Files/PLAYER-REGIS_MED%20AUTH%20FORM.doc> and fill it out at home and then bring it to the orientation. We will need an original signature on each copy of the registration paperwork. We will also have blank forms at the registration desk.

2 copies of each players birth certificate. It is much easier for us logistically for each player to bring two copies of their birth certificate, even though your last years manager has a copy already. So thanks in advance for bringing that.

For new players, we need to verify an original birth certificate (we will not keep the original). For new players, our registrar needs to verify the original birth certificate and then we will return it to you.

2. Pictures - we will take a photo of your player for their player card

3. Finances
We will collect the sign-up fee, any uniform fee and the first month dues at the orientation. We would ideally like you to pay with three checks; the sign-up fee is paid to the club, dues are paid to your team account and uniform and training kit fees are paid to SLOCO soccer store. The monthly dues get paid to your individual team account.
    • The sign-up fee for each player is $85. This fee applies to both rostered and reserve players.
    • Dues GU15- are $700 for the season for rostered players and $455 for reserve players. BU17We offer a discount if people want to pay all of their monthly dues at the orientation meeting (dues are $675 if paid at the meeting, as opposed to $700 when paying in installments).
    • Dues BU15- With the current BU14/15 player pool, we will charge $80/month. In addition, when a player attends a tournament, they will need to pay an extra $35 for each tournament and when they play in the SLO Fire Invitational, they will need to pay an extra $10 for each day they play.
    • Dues BU17 & GU18- We are trying a pilot program with our two older teams. Dues are $480 for the season. In addition, when a player attends a tournament, they will need to pay an extra $35 for each tournament and when they play in the SLO Fire Invitational, they will need to pay an extra $10 for each day they play. No reserves.
    • Financial Aid complete a financial aid application if needed. Blank forms can be found here: http://www.slosoccer.com/SLOSC/documents/SLOSC%20Financial%20Aid%20App%202008.doc

4. Uniforms
SLOCO soccer store will be there to size your player for uniforms and take payment,

Accept roster position online
You need to go back to the player account you created online and formally accept your position on the team. This will allow us to build complete team rosters and know that you are willing to be part of the SLOSC for this next year. http://www.bluesombrero.com/Default.aspx?tabid=29328 <http://www.bluesombrero.com/Default.aspx?tabid=29328>
